  • Maintain accurate records and care plans in partnership with tenants
  • Liaise with families, carers and external professionals as appropriate
  • Support tenants to access healthcare, appointments and community opportunities
  • Provide guidance and support to Support Assistants, including allocating tasks and sharing good practice
  • Undertake shift work, including lone working and regular sleep-in duties

About you

You will be caring, organised and confident in your ability to support others while managing responsibility and decision-making in real time. You will be comfortable working autonomously, able to problem-solve and committed to person-centred, strengths-based support.

You will also have:

  • Experience working with disabled adults in a care or supported living setting
  • The ability to work independently and as part of a multi-disciplinary team
